Hi Alyse,
To confirm, are you referring to the Summer Sale discount? Please note that the discount is applicable only for upsells. This means the discount is only available for Silver and Gold annual subscription plans for your account. Let me show you how to upgrade:
1. On your My Forms page, click on Pricing in the blue navigation bar at the top of the page.
You can also upgrade your subscription on your account’s Upgrade/Billing page.
2. On the Pricing page, choose the plan you want to buy.
3. In the popup window, choose either a Monthly or Yearly payment schedule and either Credit/Debit Card or PayPal as the payment method.
Here’s an example of the Bronze plan:
After you click on Continue, you'll need to enter your Credit/Debit Card details.
If you choose to use PayPal, you’ll get redirected to the PayPal payment page to complete the payment. Check out this guide for more details about upgrading your account.

So, what would happen to the amount that I paid for the bronze package? Is it credited to this amount required for the upgrade?

I've paid for the year for the bronze package not knowing my forms would be unavailable. I would not have paid for this subscription if I had known.

Hi Alyse,
I have reviewed your transaction for the Bronze Yearly subscription plan and the 50% discount was applied to it. You can review the invoice under your account on this page. As for your forms, currently, they are disabled due to HIPAA. If you are not a HIPAA covered entity, you can confirm it via the Data page. Once confirmed, the system will automatically enable back your forms. Here's how to do it:
1. In Data, a pop-up modal will appear.
2. Click on No, I'm not a HIPAA-covered entity.

Hi Alyse Carter,
I've checked your two forms are disabled. You will need to enable them from your form builder. Let me walk you through the steps:
- Open your form in Form Builder.
- In the orange navigation bar at the top of the page, click on Settings.
- In the Form Status, select Disable option.
- In the pop-up window, choose Enable. That's it, you're done.
Here is a helpful guide on how to enable the forms. This should resolve an issue.

I guess it just needed a few minutes to refresh because I'm able to edit my forms again.
