Each submission is coming through twice, automatically, even though the submitter only filling out the form once.

Thanks for reaching out to Jotform Support. When I cloned and tested your form, everything worked the way it was supposed to. Take a look at the screenshot below to see my results:
I checked your Jotform Tables, and could confirm that there are submissions that have duplicates. I've gone ahead and cleared your form caches. If the issue continues to happen, I suggest enabling the Unique Submission feature on your form based on either their IP address and cookies, or their email address. Let me show you how to do that:
- In Form Builder, click on Settings in the orange navigation bar at the top part of the page.
- Under Form Settings, click on Show More Options.
- Scroll down to see the Unique Submission dropdown, and select Check cookies and IP address.
- If you want to limit one submission per email address, select the email field from the Unique Field dropdown.
