Autoresponder is not working, and notification emails does not come with PDFs

    Hi mam/sir,

    We have a problem concerning our form for requesting a MATI Doctor Referral Slip. We are receiving no responses to our requests, or we are receiving responses with no PDF attachments.

    Hi employeehealthcenter,

    Thanks for reaching out to Jotform Support. I checked your form and see that the PDF report has not been selected on your email alerts yet. Let me show you how to do that:

    1. In Form Builder, click on Settings and Emails.
    2. Next, click on the Pen icon over your Notification Email.

    3. Under the Advanced tab, select the PDF template you'd like to attach and click Save.

    About your other question, I see that there are conditions associated with your autoresponders which could trigger them conditionally. Depending on the submissions, the autoresponders may or may not be sent. If you can share with us the affected submission ID and the email address you expect to receive the emails, this will allow us to investigate and see what's happening further.
