Can I populate a check box in a Word Template?

I would like to know if it possible to populate check boxes in a word template from check boxes in a form. I have previously set up a form that populates a word template using Make but is there functionality to populate check boxes as well?

When setting it up, there will be a step where you can map your form fields to your Word template.  Take a look at the screenshot below:
