Form: Why is the embedded form asking people to request access?

Thanks for reaching out to Jotform Support. The reason why your form is asking people to log in is because your form is set to private. If you want everyone to be able to access your form, you can update the status from Private to Public. Let me show you how to do it:

1. In Form Builder, click on Publish from the top bar menu.
2. Next, click on the Private Form button and select Public Form.
