How to allow the users to add a new set of fields for another person's details?

    Basically, I need to have a field that will allow them to enter their name, age, and email. Show a link to our waiver agreement, and then a field to have them sign their name.


    Then I need an 'add new' button so they can fill out all the members in their family participating and agreeing to the waiver.

    Thanks for reaching out to Jotform Support. If you need to allow the users to add a new set of fields for the details of another person, you can use the Configurable List widget. The users can add as many rows or sets of the same fields as needed for new entries. Let me show you how to add it to your form:

    1. In the Form Elements window, go to Widgets.
    2. Drsg and drop the Configurable List widget to your form.
    3. Customize the fields in the widget.
    4. Click on the Update Widget below.
