Hi Kap,
Thanks for reaching out to Jotform Support. After checking your account, it appears that it has been suspended because one of your forms was requesting for usernames and passwords. Please be note that requesting login credentials violates Jotform's Terms of Use. I have removed the offending fields and reactivated your account. Please log out and then log back in. Everything should now work correctly.
