How to format options in Dynamic Dropdowns Widget?

It is not displaying correctly on my form.

How can I solve it?

Thanks for reaching out to Jotform Support. I checked your formatting of the options as they were not formatted properly. You need to add SPACES to create a parent-child relationship. You can use this format of options and paste it as it is in your Dynamic Dropdowns widget Value's section:
