Forms are disabled due to HIPAA.

    my business in not required to be hipaa compliant please re-enable my form

    Hi EMSInfo,

    Thanks for reaching out to Jotform Support. If you're not a HIPAA-covered entity, you need to select the option 'No, I'm not a covered entity' from the HIPAA modal shown in the screenshot below:

    Let me show you how to do that:

    1. On your My Forms page, click on your Profile Photo/Avatar, and select Settings.
    2. Go to Data, and select No, I'm not a HIPAA covered entity if the modal pops up.

    But, if you run into the pop-up similar to what's shown in the screencast below, just click on "Go Back", then you'll see the modal, from which you need to select "No, I'm not a HIPAA-covered entity". Check out the screencast below:

    If the modal still isn't showing, try logging out of your account, clearing your browser cache and cookies, and then logging back in. Afterward, try going back to the Data section of your account. Once successful, your forms will be enabled.
