Conditions not working.

I am having issues with the conditions on page 3 for the question "What type of MOD work are you requesting?"

After checking your condition, I noticed that 2 conditions in your form caused the conditions you have set up not to work. You just need to remove the conditions shown in the screenshot below:

Let me show you how to delete it:

1. In Form Builder, in the orange navigation bar at the top of the screen, click on Settings.
2. Select Conditions on the left. 
3. Next, delete or disable conditions 18 and 22 by hovering your mouse on the gear icon.
