How to upgrade to Bronze monthly plan for $19.50?

Hi, I'm creating a form for my church camp and we have used Jot form before but it is very expensive to upgrade. We would like to keep using the form builder as it works well for us but we are only a small church and the monthly price for the bronze plan is way too much for us. I noticed there is a sale of 19.50 for the month. We will be needing it until the end of August for our camp so we would like to upgrade for the next 2 months and pay the monthly price of 19.50. if this is not an option we will have to look for another form builder.

Thank you, its saying If you are outside the United States, please attach the appropriate document that officiates your nonprofit status. I'm not sure what it's asking for exactly can you please be more specific? We are a church and our website is ctkginsons.com. Is that sufficient?
