Not getting emails once form is submitted

Thanks for reaching out to Jotform Support. I'll do my best to help you. Can you please share with us the form URL in question for which you are not receiving the notification emails? Are you referring to the ACB/ACFC/ACMC Prospect Screening Form? If yes, then when I cloned and tested your form, I could not replicate an issue. Let me share the screenshots below with the results:
On this form you have set up Change the Email recipients, so, do note that the notification email will only be triggered to the set email address when the condition is met.
Your following email addresses are not on our bounce list, either.
- hgrant@allcountyboulder.com
- hlynch@allcountyfc.com
- chadd@allcountyfc.com
- ckienitz@allcountyfc.com
- cdubois@allcountyfc.com
- haydenp@allcountyfc.com
You should add these two email addresses to your contact list:
To resolve an issue in the future, you can use your own server for sending the notification email. You can also whitelist Jotform's IP addresses and domains. These are the IP addresses:
- 152.160.0.0/16
- 23.251.253.0/26
- 23.26.226.0/24
You can refer to the guide on how to whitelist Jotform's IP addresses, and this guide will show you several reasons why you are not receiving the email notification. You can also set up your own SMTP. Sometimes, removing and adding back the notification also resolves an issue.
