I’m having issues with the form loading for my customers. Are you having server issues?
Specifically my work authorization form (web)

Thanks for reaching out to Jotform Support. There aren't any reported server issues today, and as for our status, all should be working as expected. Feel free to check out this page.
Also, I tried to check your form to see if I could replicate the issue, but everything is working as it should. I was able to view the form normally without any loading issues:
