Hello Casanova_Angela,
Thanks for reaching out to Jotform support. The error This form is currently unavailable! shows up when the form is disabled. Let me show you how to enable it:
- In your Form Builder, click on Settings from the orange tabs.
- Click the Form Status from the left panel.
- Change form status to Enabled from the dropdown and click Save.
Give it a try and let us know if you have any other questions.

Hi Lara,
Thank you for your quick response. This definitely looks like a solution to get the form to appear.
However, the issue stems from I don't even know how the form got on the site to begin with. I didn't add it, and I don't formally have an account with jotform before this issue.
Any other ideas how I could remove the jotform template all together?

Hi Angela,
I saw that this form is under an account with the username "tgatexas." Also, the email address used to create this account is of***e@t**te**s.o**. There must be someone in your organization who created an account on Jotform.

Hi Gian,
Thank you for replying. I just want to confirm that the organization email used has four characters before the "at sign" and ends with an "e"?

Hi Angela,
The account email has 6 characters before the @ sign. It starts with an O and ends with an e. I have sent a password reset email to this account email address.
