HIPAA-covered entity: Form is disable

    Hi, my forms are locked although I am not a HIPAA covered entity. Thank you.

    Hi Hailey,

    Thanks for reaching out to Jotform Support. If you are not a HIPAA-Covered Entity please select "No" in the HIPAA modal in your Account Settings. Let me show you how:

    1. Click your Profile Avatar.
    2. On the dropdown menu select Settings.
    3. Select Data on the left side menu.'HIPAA covered entity: Form is disable Image 1 Screenshot 40
    4. Then, select "No, I'm not a HIPAA-Covered Entity in the modal".HIPAA covered entity: Form is disable Image 2 Screenshot 51

    If you didn't see the modal in your account settings, please sign out to your Jotform Account and sign in again. The HIPAA modal will appear and select "NO, I'm not a HIPAA-covered entity". Then, you can now enable your form. Let me show you how:

    1. In Form Builder, click Settings on the orange navigation bar.
    2. Under the Form Settings, hover your mouse and click the Form Status of the form.
    3. Select Enable and click Save.

    Thank you. I have done this but my forms are still locked.

    Hi Hailey,

    After checking the forms, I was able to confirm that it's now active and can be accessed anytime. I cleared your form's cache for extra measure. Can you try again and see how it goes?
