i had created form but now it is not there

i had created 2 forms but now i can not see them in form but one of them is in

approval page

please explain the changed that happened

Thanks for reaching out to Jotform Support. I took a look at your form and found there's only one form present in your account, which isn't enabled yet.

And the form you're seeing on the approval page isn't actually the form, it's the approval flow you created in your form. You can access that approval flow from the approval page as well as from your form.
