Hi,
My account is suspended, and I would like to reactivate it. I'm unsure why this happened. Please help me resolve this issue.

Hi RE,
Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Protected: Staffing Requirement - FE that’s breaking our Terms of Use by collecting login credentials.
I've deleted the offending fields and reactivated your account. You should be able to log in now.
