    Hi samuel,

    Thanks for reaching out to Jotform Support. I checked your account and it's currently suspended because it has been flagged as a fake service. To reinstate your account, we have to delete the offending form along with its submissions if there are any.

    If you think you have been flagged incorrectly and don't want to delete the form with its submissions, send an email to reviewers@jotform.com with the subject Thread #16209191. In your email body, provide the following:

    • The URL where your form is used.
    • Any documents that prove that you are authorized to offer such services.
    • Your website or proof of affiliation to a legitimate institution.

    We will let you know about the status of your account after a thorough review of your proofs.
