Suspended account: Fake services

Hi,

I am setting up a website for my client'sLLC. I am using jotform to obtain necessary information that is required for credit applications. The business purpose is to lease/buy/sell new cars.

How can I re-activate my account ASAP? If you need any documentation , please let me know.

I need this website open for my client, I am transitioning the website to his possession by end of day.

Thanks for reaching out to Jotform Support. After checking your account, it was suspended because your form was flagged for offering fake services. We had to act according to the general rule to comply with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. You need to provide us with the following information:

• The URL where your form is used.
• Your website or proof of affiliation to a legitimate institution.
• Change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

Send these to reviewers@jotform.com with thread #16204211 as your subject, and we’ll let you know about the status of your account after we’ve done a thorough review of your proofs. We also need to remove the offending fields such as the collections of driver's license and SSNs before we can reactivate your account. Let us know if you wish to proceed.

Well my client needs to obtain SSN and photo of driver's license for the credit app.

Cause if thats the case I will have to find another program.

I have all of the requested items and the company domain email. But there's no point in continuing for me to try to re-activate this account if we cant collect the required info. My client cannot pull a credit application from his customers without a SSN and DL photo. Is this feature allowable in any other plans? Do i need to buy a business plan instead of the free or?
