    Hi Amal,

Thanks for reaching out to Jotform Support. Please confirm whether you are a HIPAA-covered entity. If so, you are required to upgrade to a HIPAA account under our Gold Plan. If not, you can follow these steps:

    1. Sign out of your Jotform account and Sign back in.
    2. Once you're signed in, you will see a modal like in the screenshot below. If you do not see it when you log back in, you can go to your Data Settings, and click on Go back when you see the modal that pops up.Forms are disabled due to HIPAA Image 1 Screenshot 40 Screenshot 20
    3. Then click on "No, I'm not a HIPAA-covered entity".Forms are disabled due to HIPAA Image 2 Screenshot 51 Screenshot 31

Following the steps above will automatically unlock your forms. Then you need to enable your forms. Here's how to do that:

    1. Open your form in Form Builder.
    2. In the orange navigation bar at the top of the page, click on Settings.
    3. In the Form Settings window, set the Form Status to Enabled and click on Save.
