Hi Ryan,
Thanks for reaching out to Jotform Support. I checked your form and I saw forms in it. If the form and app are not listed in your All Forms folder, can you share the link to the form and app you're referring to so that we can assist you better and respond accordingly?
Once we hear back from you, we'll be able to help you with this.

Hi Ryan,
After checking the QR code you shared, I saw that the app that you're referring to was created from a different account. The account is associated with the in*o@co****es-d***ct.co.uk email address and is using Karl_info863_info as the account username. Please log in to the correct account for you to access the form and app.
Let us know if there's anything else we can help you with.

Hi Karl,
The email address associated with the account with username Karl_info863_info is in*o@co****es-d***ct.co.uk. If you are familiar with this email address, you can proceed with using it in logging in to the correct account. And, if you don't remember your password for this account, you can reset it. Let me show you how:
1. Go to the Jotform Main Page and click on Login.
2. On the login screen, click on Forgot Password.
3. Enter your email address and then click on Send Reset Instructions.
4. You'll get an email with instructions about how to reset your password. In the email, click on Reset Password.
5. Enter your new password, enter it again to verify it, and then click on Create a New Password.
Your new password is now set, and you can use it to log into your Jotform account.
If you do not recognize the email address, please ask anyone on your team or your IT Department to give you access to that email, so you can access the form again.
Reach out again if you have any other questions.
