Form Locked: required to upgrade to HIPAA

    I cannot access my form because it has been flagged as a form that involved health care providers. I am a personal trainer and do not collect any data tied to HIPAA. Please unblock my form so I can have access to my client responses.

    Hi Alex,

    Thanks for reaching out to Jotform Support.  A HIPAA-covered entity is an individual or organization that must comply with HIPAA requirements. These include healthcare providers, insurance companies, clearinghouses, doctors, dentists, pharmacies, and other entities that handle protected health information.

    If you do not collect data tied to HIPAA, you must select the "NO I'm not a HIPAA-covered entity" option in the modal. Take a look at the screenshot below:

    You can also open the Data section under the Account Settings to turn it off. Let me show you how:

    • Login to your account, and click your profile icon at the upper right part of the page.
    • Then select settings in the menu.
    • Click data on the left panel then the HIPAA Enforcement modal should pop up. If there's no pop-up, please Clear the Cache and Cookies in Your Web Browser.

    I tried to do both of these...it still does not allow me to access the form

    Hello Alex,

    HIPAA lock is now been removed from your form, and I also cleared your form cache, can you give it another try and let us know what happens? If you're still unable to access your form, we'll do a more in-depth investigation and work on finding a solution.

    I tried both methods and I still cannot access my form. It's still grayed out.

    Hello Alex,

    Yes, I see that your form has been disabled again. Before reaching out to the Relevant Team, can you confirm that you are not a HIPAA-covered entity, could you please respond to this thread with "I am not a covered entity under HIPAA"? 

    As soon as we hear back from you, we'll be able to help you with this.

    I just checked your account and your form is not locked anymore. I was also able to access it already. See my screenshot below:

    If you still can't access it, try using another browser or device.
