Hi,
Ive tried to research the answer to this but couldn't find what I was looking for.
Ive created a landing page in jot form. Please see form here:

On this particular landing page I want to remove the current fields asking for name email etc and put them on a seperate landing page that links to this one.
To be more specific, on the form, if you scroll toward the bottom, where the heading asks: ARE YOU READY TO HELP THIS GREAT CAUSE I would like a yes button and then take the customer to a seperate page to fill in their details (Name, Email etc).
Ive also attached a screen shot.
I would be so grateful if you could explain how to do this.

To do that, firstly you can create a single choice field.
Then hide all fields which you don't want to see on the first page:
After that, go settings and add Show/Hide conditions.
