Hi Luzmarina,
Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named "Credit Card Transaction Form" that’s breaking our Terms of Use by collecting credit card information in plain text fields.
In line with our PCI certification, we have to deny the collection of credit card details on regular forms. But, we have other PCI-compliant ways you can do this. All you need to do is choose one of the payment processors available on our platform and integrate it into your form. You still won't be able to collect credit card details directly, though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of our Payment Authorization feature.
If you’ll agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can lift the suspension on your account.
Once we hear back from you, we'll be able to move forward with this.

I totally apologize, I was not aware of this... I am really sorry! My
intention was never break your terms and conditions.
Yes, I agree on these terms! Can I link my Square account to take payments
in that case?

Hi Luzmarina,
I've now deleted the offending fields and reactivated your account. You should be able to log in now. Also, integrating a Square account with your form is easy. Let me walk you through it:
1. In Form Elements, open the Payments tab on the top-left side of the page and then click on Square.
2. In the Square Integrations field on the right side of the page, click on Square Connect to connect your Square account (if you want to do a test, choose Test Mode).
3. Next, scroll down and click on Additional Gateway Settings.
4. Toggle Authorization Only to the On position.
5. Now, just click on Continue and that's it. You're done.
You can also check out our guides about How to Integrate Form with Square and How to Enable Payment Authorization for more detailed information.
Reach out again if there’s anything else we can help you with.

Thank you so much for reactivating my account and guide me through the
process
If I need to request customers credit card information as a method of
deposit, which form should I use? Can I link that form to my contract for
them to fill out by the time the contract is signed?

Hi Luzmarina,
We don't allow the collection of credit card details in any form, but you can choose from the below payment gateway, and enable the Payment Authorization feature.
As mentioned by my colleague, this will allow you to collect your customer's payment details and charge them at a later date.
Reach out again if you have any other questions.

Hi Luzmarina,
If you don't intend to charge the customers right away, you can enable Payment Authorization as my colleague Angeli mentioned in her reply here. In that way, you can get their payment details and charge them at a later date.
Give it a try and let us know if you have any other questions.
