Embed my form into my APK website.

Hi,

I'm looking to create a form on my APK website to collect reviews specifically about download speeds. I want users to be able to rate their download experience and provide feedback on how we can improve. Has anyone implemented something similar and can share their approach or recommend any tools or plugins that would make this easier?

For context, my website hosts the Loklok apk, visit and reply me your valuable thoughts . I’d like to ensure users have a smooth and quick download experience and gather data to help make improvements.
