How to embed form to ShowIt website?

Where in the world do I embed the form code on my ShowIt website? The form I created will go in the middle or toward the bottom of a page on my site. I don't want the user to have to leave my website to fill out a form.

I'm trying to upload a screenshot but it says Invalid file name.
