Hi Carrie,
We advise you to access your account and confirm that you're not a HIPAA-covered entity. Let me walk you through it:
- On the My Forms page, click on your Profile/Avatar in the upper right corner.
- Select Settings in the submenu.
- Select Data in the left corner.
- Next, you'll encounter the HIPAA Enforcement modal. In this modal, you should select the "NO, I'm not a HIPAA-covered entity" option. Once you select this option, your form will be enabled.
Give it a try and let us know if you need any help.

Hi Carrie,
You can access the pop-up modal through your Account Data page, following the steps outlined by my colleague Jason. Another option for resolving this issue is to log out, log back in, and then follow these instructions.
- On the My Forms page, click the Avatar icon on the upper right side.
- Select Settings, and click on the Data tab located at the left.
- Then select NO, I'm not a HIPAA-covered entity in the modal.
If your form is still disabled after following the steps mentioned above, I suggest enabling it manually. Let me show you how:
- In Form Builder page, select Settings located at the top orange menu bar.
- Under the Form Settings tab, scroll to Form Status.
- Change the Status to Enabled.
- Finally, click on Save.
Feel free to visit this guide on How to Enable or Disable a Form.
Give it a try and let us know if you need any help.

Hi Carrie,
After checking, it seems that the system have identified your form as HIPAA-compliant. If this is not the case, please try to do the following:
- Log out from your Jotform account and clear your browser history.
- Then, log back in, and you should then see the modal below.
- Next, click the Go Back button.
4. You should see the modal below, select No, I am not a HIPAA Covered Entity.
Once you select this option, your forms will be enabled, and you should no longer be required to upgrade for HIPAA compliance. But, it may take a few minutes to take effect.
Keep us updated and let us know if you need any more help.
