What is PS Payment Form?

    You sent me an email saying that you received my payment. Subscription no.:65228115.

    I have no idea what this is or who you are.

    The only form that I filled out was a form on Facebook regarding a dog food survey. They charged me $3.99 for shipping the product and that was all. I have not signed up for anything and I will not pay for anything but the 3.99 providing I get the product. If anything is charged to my credit card other than the $3.99 I will dispute it with my credit card company.

    Yes, you can contact your bank to dispute any further charges or the other option is to contact the form owner to verify the subscription through the Autoresponder email you've received when you completed the form. If you still have the link to the form, you can forward it here. This will help us better understand how the form is set up to collect payment.

    As mentioned by my colleague, Jotform is an online builder that helps you create a form. If you happen to have filled out a form and was charged from your card, I would suggest directly contacting the form owner about this by replying to the email you received after submitting the form.
