I've attached an image to show you what I would like to do. I need a checklist of options, when a few of the options indented. How do I do this?

Hi fbcholton,
Thanks for reaching out to Jotform Support. You can use the Multiple Choice element. Let me show you how:
- In Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Basic tab, scroll down the list and choose the Multiple Choice field.
- Now, just drag and drop it to your form.
Another option is to use the Checklist widget. Let me walk you through it:- In Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Widgets tab, enter Checklist in the search bar, and drag and drop the widget to your form.

I tried those. I need to have the following.
- General
- Registration Form
- Newspaper Ad
- Request for printed materials
However under the registration form, I need 3 options but want them lined up under Registration and not lined up with all 4 points.
- General
- Registration Form
- Registration for event has deadline
- Prefer printed registration form
- Newspaper Ad
- Request for printed materials
Can I do this?

Hi fbcholton,
Unfortunately, it is impossible to show other selection options in the Multiple choice element. I would suggest adding another multiple-choice or single-choice element to show once they select the Registration Form (see options below). Let me show you how:
- In Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Basic tab, scroll down the list and choose the Multiple Choice field.
- Now, just drag and drop it to your form.
Then, you need to create Show/Hide conditional logic to show the other Multiple choice element once they select the Registration Form. Here's the conditional logic:

You can also try to use the Dynamic dropdown widgets that might fit your needs.
