What template is fit to creating an event using app?

Hi! I am looking to utilize the app function for an upcoming event. I want to be able to have our guest scan a qr code, go to the app, input their info, and then either answer a question about each room they go to, and get a "checkmark" or "star" something like that which indicates they've been there - then submit to win prizes.

Do you have any idea which template might work best for this?

Thanks for reaching out to Jotform Support. You can try to use the Event Registration Template using the Jotform App. You can customize the template to fit your desired requirements. Also, you can share the form using a QR Code to be scanned by the guest and directly go to the Jotform App to fill out their information. Let me show you how:

1. In App Builder, click Publish on the purple navigation bar.
2. Under Quick Share, go to the Share App section and click +View More.
3. Then, select Share With QR Code, you can choose either Download the QR Code or Copy the Code.
