Hi, I am currently building a Symptom Checker form, which enables the customers to choose the radio buttons that best describe their answer for each symptom they might currently be experiencing. The answers they can choose are in columns (Not at all, A little, More than a little, A lot, and Severe)
What I want to do is to tally up all the answers they have under each of the columns.
How can I use the form calculation feature to this goal?
Kind regards, -

Hi Kim,
Thanks for reaching out to Jotform Support. Yes, it is possible however, you need to have a Column Types in a Textbox. I'll show you how to do that:
1. First, create the Input Table and change the Field Types to Multi-Type Columns.
2. Next, we will add the Calculation Values for each column.
3. Then, we will add the Conditional Logic to Update/Calculate the Field. In this process, every time they select an option, the score will show in the Score column. Check the screenshot below for the conditional logic as a reference:
4. Add another condition for each column and do that same condition as you see above for the first column. Check out the screencast below to see my results:
These are the guides that you can check:- Performing Calculations With Input Table
- Calculate Values in the Input Table Through Condition
- How to Perform Form Calculation Using a Widget

Hi Wilson,
When I opt for Multi-Type Columns this is what it shows:Which doesn't look good and confusing for the user, do I need to re-do everything as I only imported the table from my old form?

Hi Kim,
You may want to set the Input Table fields to Checkbox. This option will allow you to calculate the value using the Form Calculation widget. I can walk you through setting that up:
- In Form Builder, select the Input Table and click on the Gear icon.
- Under the Options tab, in the text box under Calculation Values, set a value to each of the radio buttons.
- While you're in the Input Table's properties, click on the Fields tab.
- Set the Input Type to Checkbox.
- While in Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Widgets tab, search for Form Calculation, drag and drop the widget to your form.
- While in Form Builder, in the blue navigation bar at the top of the page, click on Settings.
- Click on Conditions in the menu on the left side of the page.
- Select Update/Calculate Field.
- Set the condition just like what you see in the screencast below:
- Click on Save to save your work.
If you do not want the widgets to be displayed to your form respondents, you can set them to hidden. Below is a screencast of the expected result:
Give it a try and let us know if you need any help.

Jotform is so user-friendly and easy to use, that I made many forms that took only a few hours. I am now creating a timetable for my classroom but I can't find some blocks to them on the table. With the help of , I was able to track my work. It would be great if I get the video instructions for it.
