Hello,
I created a survey form. It says that it is locked because of HIPPA compliance. The form does not have any medical/personal identifiers and is just a user satisfaction survey. Can anything be done to unlock it?

Hi Interprofessional,
Thanks for reaching out to Jotform Support. If you are not a HIPAA-covered entity, please follow the steps below:
- On the My Forms page, click the Avatar icon on the upper right side.
- Select Settings.
- Click on Data on the left side.
Lastly, select NO, I'm not a HIPAA-covered entity in the modal.
If you do not see the modal, please sign out of your Jotform account and sign back in. Once you're signed in, you will see the pop-up modal. Just click on "NO, I'm not a HIPAA-covered entity" then enable your forms. Here's how to do that:
- Open your form in Form Builder.
- In the orange navigation bar at the top of the page, click on Settings.
- In the Form Settings window, set the Form Status to Enable and click on Save.

Thank you for your response. It does not show an option to select "no" and the form itself is now locked so I cannot go into edit the form's settings. I also selected "No, I am not a HIPPA entity" when it popped up, logged out and back in and nothing has changed. What next?
