Hi Shawntae,
Thanks for your patience. I cloned and tested your form and I was able to get the Afterpay popup window and submit the form. See the screenshot below:
You can try to reconnect Afterpay from your product field; this will refresh the connection on your form. Here's how to do it;
1. In Form Builder, click on the wand icon.
2. Then click the green drop down button and select Reconnect.
3. And click on Reconnect.
