    My account was suspended. How do i fixt it

    Thanks for reaching out to Jotform Support. I checked your account, and it appears that your form 241621764650051 was flagged as having a copyrighted logo. If you believe that your account was suspended in error, we’ll need you to do the following:

    • Send us the URL where your form is used.
    • Send us a link to your website or proof of affiliation with a legitimate institution.
    • Change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these things above to reviewers@jotform.com with Thread# 16050011 as the subject, and we’ll let you know the status of your account after we’ve done a thorough review of them.

    In the meantime, let us know if you need any other help.

    Hi Nicky,

    Your account was suspended because you’ve got a form named Form that was flagged for using a copyrighted logo. We understand this might not be the case with your form, but we had to act according to the general rule to be in compliance with our Terms of Use.

    For us to reactivate the account, we’ll need your permission to delete the offending form. You can create the form again after changing your account email address to a corporate email associated with the company that owns the logo. 

    Once we hear back from you, we'll be able to move forward with this.

    Okey you can delete the offending form that was flagged for using a copyrighted logo. And We will change our email once our account is unsuspended.

    This following information:

    • Type of Card : Visa
    • The last 4 digits of the Card : 9501
    • Expiration Date : 05/28
    • Date of the last charge : May 24th, 2024
    • Amount Charged: $39 USD

    We have deleted the offending form and your account is now active. Please log in and reset your password. Let me show you how:

    • Enter your username or email, then select Send Reset Instructions.

    • Check your email for the password reset instructions.
    • Select the Reset Password provided in the email.
