Copied Form did not maintain format of the PDF in the PDF editor

Hello,

I copied this corm from my personal account to my business account so that i could distribute to my employees but theres a problem. The form did not copy over the formatting of the PDF in the PDF editor. Is there any way to change that or could you copy the formatting over for me? It took me a long time to perfect and i just wanted to copy the whole form including the backend interface over to the new account.

If your copied form isn't live yet, I've gone ahead and transferred my copied form to your account. Here's the link to its Form Builder, so you can do the necessary changes. It should be visible in your account's My Forms Page as well.
