Thanks for reaching out to Jotform Support. Can you tell me more about the issue you're having? Are you having trouble creating a CME form? If so, feel free to check out our templates to see something that'll fit your need. Or if you'd like to manually create your own, let me show you how:

1. On your My Forms page, click on Create Form at the top left side of the page.
2. On the Create a Form page, click on Start From Scratch and then click on Classic Form to create a new form with the Classic Layout.
3. Click on the Add Form Element button at the left part of the builder page to start adding fields to your form.
