Heather and logo does not show

I'm having some difficulties with the forms that I have created. I have tried to share a link and have successfully had the forms submitted back to me by fellow employees via their iPads, however when i try to save the form that contains our company logo and the header that describes the title of the document, it does not save those two important elements to the copy. So I have reached out and was told I needed to convert the form to a PDF, share, and have the form filled that way. That has not worked since the form is not fillable and no able to be completed on an iPad. I was told today that I needed to covert that form to smart pdf form. So I did do that, tried to sharing, completing, and submitting- that still does not show the two important elements that need to be on the form that is either downloaded and saved or printed. Can you please help? I'm not sure what I am doing wrong. THANK YOU!!!!

Thanks for reaching out to Jotform Support. Upon checking, it shows that the form contains the header and the logo of your company. I cloned and tested your form to see if I could replicate the issues that you have mentioned, but everything is working properly. I was able to submit the form without any problem. Here's a screenshot of the sample submission.

Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
