Why account was suspended?

I was creating a form used to set up technology for real estate brokerages and my account was suspended. Can you help please?

Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Form that was tagged as there are three offending fields on your form. Do note that it is against of Jotform's terms of use to collect the username and passwords using the plain text fields. Let me share the screenshot below. 

Since, there is zero submission on the form, I've deleted the these highlighted above fields and reactivated your account.
