Embedded form is not visible on my website

Visitors not able to see embedded form on website. screenshots attached. Patron is overseas, though so I'm not sure if that has any effect or if it's even the form.

Can you tell us on which country they're from? Can you also ask them on which device they try to open the website?

He was based in the UK, London to be exact, and he tried on both a cellphone and laptop and neither worked for him.

Additionally, I've also tested it on Android phone with Google Chrome browser, and it's working as expected. Check out the screenshot below:

Can you clear your browser's cache and give it another try? You can check out this guide to learn about how to clear browser cache. If you ran into the same issue, we suggest you to share some additional information about your browser application and operating system you are using. It is also suggested if you could provide us the Browser console log report.

Once we have a better idea of what’s going on, we’ll be able to come up with a solution.
