Thanks for reaching out to Jotform Support. I checked your account and saw that it was autosuspended by our Phishing Detecting System. After checking your form, it looks like there's no other fields that are against our Terms of Use, so I've reactivated your account. You should be able to log in now. Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
