Name field is showing on top of the PDF file when downloaded.

Our ach form is not keeping the parent name on the line of the form when we down load it? See attachment.

Thanks for reaching out to Jotform Support. I cloned and tested your form, but everything was working properly as it should. The Parent's name as entered on the Full Name on Account field is mapped correctly on your form and is reflected on the downloaded PDF file. Here's a screenshot of my result:

I also checked your form and can confirm that the fields are mapped correctly on the source PDF file. As a precaution, I have cleared your form's cache, to refresh your forms. This normally fixes temporary issues. Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
