    Hi.

    I have created a form for lost property to be reported and it has been disabled.

    Can this please be reviewed so we can send this form out?

    Hi Liz,

    Thanks for reaching out to Jotform Support. Your account was suspended because you’ve got a form named Mt Hutt Lost Property Report that’s breaking our Terms of Use by collecting login credentials.

    If you agree to follow our Terms of Use and let us delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.

    Once we hear back from you, we'll be able to move forward with this.

    Hi Jeric, Thanks for letting me know. To confirm, this isn't login credentials, it simply allows us to link the lost property item to our customer in our database so we can ensure that we get it back to them. We already have this information, it will just allow us to let the customer know when they are up on the ski field that we have their item. ​ ​Now you know what this is, please let me know if we can keep this on the form, if not, please advise how I am to remove this when the form is currently disabled.     Hi Liz,

    The form is collecting password through the field MyPass card Webcode. We will need to delete the offending field so that we can lift the suspension and reactivate the account. 

    Once we hear back from you, we'll be able to move forward with this.

    Hi Liz,

    As agreed, we have removed the offending form and restored your account. You may now login and access your forms again. If the field in question was not to collect user credential passwords, consider creating it again but using a different term.
