The email field is not displayed properly on my forms

Hi sensiblehealthinitiatives,
Thanks for reaching out to Jotform Support. I checked and see that the issue is being caused by the theme. Could you try using a new theme, then add the new theme again? Let me walk you through it.
1. On your Form Builder Page, click on the roller paint icon.
2. Go to Themes.
3. Select a different theme, then change it back to your previous theme.
Give it a try and let us know how it goes.
