How to Send the Result After Completing the Form?

How do I send a report automatically to a customer who has completed their online quiz, based on the answers to the quiz ?

Thankyou for answering my message so quickly.

I have created an online quiz that customers can answer with basic A or B answers (represented by the colours GREEN & RED). I want to send a simple visual report to the customer based on their answers that shows the percentage of how many A answers Vs B answers to determine their metabolic type.

When I go into the report section on Jot form I’m not entirely sure how to get the form to show the customers answers plus then send them their report.

Are you able to assist me please?

Unfortunately, you can't send or generate a visual report for each customer individually. I understand what you’d like to do, but I’ll need a bit of time to work out a solution. I’ll get back to you shortly.

You can easily get the total of the green and red answers by creating an Update/Calculate field condition in which a value of 1 is inserted into the greed/red field that is hidden below the questions if that specific option is selected. Then, we'll use 2 Form Calculation widgets, one for the total of green answers and another for the total of red answers. Let me show you how:

• Include a unique name for each field, as I did below with Q1 for Green and Red of Question 1, to make it easy to identify the fields when creating conditions. 

You have to create the same type of condition for each of the form's questions. As an example, I just used the first choice for green and the second option for red below. 

• Set up an Update/Calculate field condition to insert a 1 value if a specific option is selected in the questions. To learn more, see How to Insert Text or Calculation Into a Field Conditionally guide.

Here's what the conditional logic for Question 1 looks like if you selected the first option:

• Next, add 2 Form Calculation widgets, one for green and one for red. Then, make sure to add the fields for Green and Red to each widgets. Then add a plus sign (+) between each field. 

• You must disable the Calculation Values for all 14 questions. 

• You should also remove the conditions in your form you have before because you will no longer need them. 

Next, generate 2 Autoresponder emails. One for the email if the Green is greater than the Red and includes the metabolic type, and one for the email if the Red is greater than the Green. Because we will set up a Change Email Recipient condition to send the appropriate email based on the results. 

• Add 2 autoresponder emails, rename each one, then edit emails.

Here’s my cloned form to see how it works. Please enter your email address to receive the autoresponder. Note that all of the first options are the green, while the second options are the red. You may also clone my form using its URL to have the exact form in your account, to see the form builder, conditions, fields. This guide will show you how to do that. I also recommend adding another question to make them 15 in order to prevent to having a same results.
