Hi, is it possible to create an image field that when filling / loading the image it will appear on the PDF?

Hi Dan,
Thanks for reaching out to Jotform Support. Yes, you can add an image field and upload the photo you want to display in the form, and this image will also appear in the PDF. Let me show you how:
- In Form Builder, click on the Add Form Element menu on the left side of the page.
- Under the Basic tab, scroll down the list and choose the Image field.
- Click the Choose a File and select the image.
Take a look at the screenshot below to see my results in PDF:
However, if you want your users to do the upload and display the results in the PDF. You may use our Image Upload Preview widget to allow your users to upload an image and preview it while filling out the form, and the image will also be displayed in the PDF. Hi Dan,
You can put the images in the form, but they will all appear in the PDF. However, you can always edit your PDF to choose which fields to display, or you can upload the image directly to that submission's PDF. Let me show you how:
- In PDF Editor, select Add Element in the upper-left corner.
- Select Image.
- Click your Image's gear icon to open the image settings.
- Click the Upload File and select the image.

Hi Dan,
I cloned your form and tested it to see if I could replicate the issue, and I ran into the same issue. When I looked at your form, I saw that the Image Field can't be mapped into the original PDF.
Instead, you can use the Image Upload Preview Widget. Let me show you how:
- Click Add Form Element, on the left side of the page.
- Select Widgets.
- Search Image Upload Preview, then select.
If that’s not exactly what you’re looking for, can you explain a bit more, so I can help you better?

I cleared the cache on your forms, can you try again? If the issue persists, will you allow us to add the Image Upload Preview widget to your form?

Hi Dan,
I have successfully added the Image Upload Preview widget on your form. Let me show you how to Map it to the original PDF.
1. Click on the Image Upload Preview widget, click on the Add to PDF button.
2. Mapped it to where you want the image to show on the PDF.
