Issue with embedded form in Blogger

I previously had a Contact Us form on this blog and it worked fine. I noted today that when the Contact Us tab was selected, it produced an error that stated form was not available. I found that I had deleted that form to work on something else, so I created a new Contact Us form.

As in the past, I used the 'embed' code and pasted it into my Blogger page .. got an error. When I preview the page it looks great, so I went back and tried it on several browsers, errors.

I used the I Frame method and when I select the tab, I am getting a 404 error. Not sure what I am doing wrong.

Can you go ahead and try it in a different browser and let us know how it goes? If the same thing happens again, we'll investigate it more to see if we can figure out what's going on.

Please click on the 'Contact Us' button and see what you see. It gives me a 404 error on Edge, Chrome, and FireFox
