Hello,
My Health Assessment Form is blocked due to the HIPAA Upgrade Request. I am not collecting data for a HIPAA-covered entity. I am a health coach with OPTAVIA and I ask that my interested clients and coaches complete this form before we chat so that I am able to know their needs and wants ahead of talking with them. Is it possible to remove the HIPAA-Covered Entity subscription block please? I utilize this form daily and need access to it, which I currently do not have.

Hi Missi,
You can go to your account settings and click on Data, a modal should prompt asking if you are collecting data for a HIPAA covered entity. Here's how to do it.
- Click on your Profile Picture or Avatar.
- click on Settings.
- Click on Data.
- A modal should prompt, asking if you are collecting data, then click on No, I'm not a HIPAA-covered entity.
Give it a try and let us know how it goes.
