How to add fields to allow users to enter company name and upload a logo?

Hi how do i add a field where someone can put their company name and upload a logo?

Thanks for reaching out to Jotform Support. You can do this by adding a short text field for the company name and a file upload field to upload a logo. Let me show you how to add it to your form:

• In Form Builder, click on the Add Form Element on the left side of the page.
• Under the Basic tab, drag and drop the Short Text field onto your form to create a field for the company name.
• Next, select the File Upload field and drag it onto your form as well.
• You can rename the fields by clicking on them and typing in "Company Name" and "Upload Logo" respectively.

Aside from the File Upload form element, you can also use file upload widgets to accept file uploads on your form. We also have a guide about How to Create File Upload Forms that you can check out.
