link to create for each of my forms on link tree

    how do i create a link for my forms using name of one of form on link tree so people can take assessment

    Hi Natalie,

    Thanks for reaching out to Jotform Support. Can you explain a bit more about your question? If you want to customize the form's URL, then you can do it from the builder. Let me walk you through it:

    1. On your My Forms page, hover your mouse over your form and click on Edit Form on the right side of the page.

    2. In Form Builder, in the orange navigation bar at the top of the page, click on Publish.

    3. Click on the Pencil icon under the Settings button

    4. Edit your form link, click on Apply, and that's it, you're done.

    Now, your form link will look like the one below:

    https://form.jotform.com/yourusername/yourformname


    Keep in mind that you can only customize the form name in the URL, the slug at the middle of the URL depends on the username of your account.
