Those fields are full in the form but in Zapier it shows all answers as empty.

Thank you for reaching out to Jotform Support. I cloned and tested your form and integrated it to Zapier and I was not able to replicate the issue that you were getting. See the screenshot below:
Can you try to disconnect and reconnect your Zapier integration from Jotform? Here's a guide how to do it:
1. In Form Builder, click on Settings.
2. Choose Integrations on the left side panel.
3. Search Zapier and click on it
4. And click the Disconnect button and confirm it.
5. Click Connect Zapier, choose an account, and try to pull up the submission data again.
